Memory Care Home Solutions™ will make it easy for you to adapt your home and interactions to meet the ever-increasing demands of caregiving. Over the course of one year, Memory Care Home Solutions™ will provide the following assistance:
CAREGIVER CONSULTATIONS: Consultations will assist you in evaluating what type of support is best for your personal situation.
- Two in depth home visits which include consultation
- Written comprehensive strategy plan tailored specifically for your family.
- Three follow up telephone family conferences
CAREGIVER TRAINING: Customized training, conducted during the two home visits, offers simple strategies to help you:
- Transform the home into a healthy, safe, and comforting place.
- Improve communication with the person suffering from memory loss.
- Modify daily activities to promote independence for the person suffering from memory loss.
- Reduce caregiver stress
REFERRAL AND AFTERCARE: Implementing changes can be overwhelming, but you are not in this alone. We are available to assist you throughout your journey as a caregiver with:
- Serve as a single access point to community resources
- Professional advice services
- Support via telephone and/or email
Service Fees
As a not-for-profit organization and in an effort to serve as many caregivers as possible, Memory Care Home Solutions charges only a minimal fee while delivering the full value. This reduced fee is made possible through donations from foundations and private donors who believe in the Memory Care Home Solutions model.
